By booking an appointment with Performance Consolidated, you agree to the following terms.
These terms are in place to ensure that treatment can be delivered safely, professionally, and within the appropriate scope of practice.
Health Screening
All clients are required to complete a health screening form prior to their appointment.
This information is used to identify potential contraindications and ensure that treatment can be delivered safely.
Clients are responsible for providing accurate and truthful health information. Failure to disclose relevant health information may result in treatment being postponed or declined.
Scope of Treatment
Sports massage therapy provided by Performance Consolidated operates within the professional scope of Level 3 and Level 4 Sports Massage Therapy qualifications.
Treatment may include the assessment and management of non-acute soft tissue dysfunction, muscular tension, and movement restriction.
The service does not include medical diagnosis or treatment of acute injuries or medical conditions.
Where symptoms fall outside the practitioner’s professional scope of practice, clients may be advised to seek assessment from an appropriate healthcare professional.
Practitioner Discretion
Performance Consolidated reserves the right to decline or discontinue treatment where it is considered unsafe, inappropriate, or outside the practitioner’s professional scope of practice.
This decision is made in the interest of client safety and to maintain professional standards of care.
Appointments and Cancellations
By booking an appointment, clients agree to the Cancellation Policy outlined on this website.
Appointments cancelled with less than 48 hours notice may not be eligible for a refund.
Client Conduct
Clients are expected to behave respectfully and professionally during appointments.
Performance Consolidated reserves the right to terminate a session if behaviour is considered inappropriate or unsafe.
